The Community Health Worker (CHW) will provide a wide range of public health services

and supports,provide accessible information, assisting with accessing vaccines (including

boosters) and connecting participant to other services,transition and diversion from highrisk congregate settings to community living, health and wellness programs, activities

that address social isolation and social factors of health, and other activities that support

the public health and wellbeing of Tribal Elders and their caregivers of the AlabamaCoushatta Tribe of Texas.


• May plan and coordinate vaccine drives, assisting the Chief Kina Health Clinic

Public Health staff and Emergency Response with vaccine information and access

to vaccines.

Coordinates and provides COVID-19 prevention education.

Conducts community outreach to educate and promote COVID-19 Vaccinations.

Provides transportation for elders and caregivers without any means of

transportation for vaccine appointments.

Delivers food and medication to the elderly.

Refers possible COVID-19 positive elders to their nearest testing center and

organizes transportation.

Call and check on the elders and their household who are in isolation with mild

symptoms, and monitor them for worsening symptoms.

Coordinate with community service providers

Documenting all activities and communications required under the Title VI grant

Assist with planning and coordinating activities with the elders.

Assists in other areas when needed including the kitchen duties, office duties,

meal deliveries, caregiver duties, etc.

Assist with maintaining a clean, safe environment in kitchen, dining and restroom


May work evenings/weekends, when necessary due to scheduled activities.

• Performs other duties as assigned.


Strong communication skills.

Ability to work independently.

Ability to work under own initiative without adequate supervision.

Ability to publicly speak in a clear and concise manner.

Knowledge of Alabama-Coushatta Tribal community resources.

Knowledge of State and County resources


Must have a High school diploma or its equivalent.

Must be able to speak and understand the Alabama-Coushatta language, but not


Must be able to travel for trainings, if necessary.

Must be willing to obtain a health card.

Must be willing to lift over 10 lbs.

Must have a valid Texas driver's license.

Must be able to pass a drug test.

Must be able to pass a background check.

Must be or willing to obtain a CPR/First Aide Certification.
